Not bad but they are not responsive design and they don't have seurity captchas which is vital for spam protection.

If anyone is interested, you can get a form like this made responsive. In other words, it looks good on all screen sizes from an iPod to a wide screen tv or projection screen.

There are many great contact forms and login and membership forms to choose from, all with security captchas.
